Please follow these simple
guidelines to an easier transition to your new health plan.

For Vision Claims – VSP Choice Network: Simply provide
the Primary Subscriber name and verify the last four numbers of the Primary Subscriber’s
Social Security No.
No insurance card will be
issued for VSP vision coverage.

Tell your doctor and pharmacy about your change
in plans.

You will need to get all NEW prescriptions for your new
plan. Your pharmacist may be to assist
you with this. If not, you may need to
contact your doctor. You cannot use
refills.

Remind your doctor to check your medications
against the drug list of your new carrier.

Check out the website www.bcbsvt.com to verify your primary
care doctor, pediatrician, OBGYN, and specialists are in the network.

If you have a non-emergency ask questions
first. Check your contract and call with
questions. Then you will have no
surprises.

When you receive it in the mail, look at your
plan benefits contract and summary sheets, this way you will know how your plan
works before you use it.

Some services do require pre-certification so
make sure to check your contract and call the insurance company to verify that
this process is satisfied properly before you have services rendered.

When you get your cards make sure you check the
spelling of all family members.

After 10/1/12, you can call the toll free number
for Blue Cross Blue Shield of VT (1-800-247-2583) to get your Subscriber ID
Number, or you can log on to the BC/BS website on 10/1 and download a temporary
ID Card:
